My law firm has recently been inundated with calls from Arizona residents requesting information about what kind of documents they should have in place with respect to COVID-19. Therefore, I have created a package of documents which can be signed and executed in adherence to the social distancing protocols currently in place. All meetings will take place over the phone or through Online Videoconferencing.
for the most part, the Last Will and Testament directs who receives your assets if you pass away. It also directs who will act as guardian of your minor children if you pass away.
This document identifies who you want to make medical decisions for you if you are unable to make them for yourself. It also answers questions regarding organ donation, consent to an autopsy and whether you want to be buried or cremated.
This document identifies your wishes with respect to removal of life support (i.e. a ventilator) if you are in a persistent vegetative state or irreversible coma.
A Testamentary Trust is language added to your Last Will and Testament which creates a trust for the benefit of minor children in case of your death. You identify the responsible person or persons who will mange the assets for the benefit of your minor children, until they reach a responsible age.
In order to execute these documents you will need TWO WITNESSES. Due to the current social distancing protocols, your witnesses will typically be TWO NEIGHBORS. You will receive instructions on how to execute your documents observing the current protocols.
